About the role

Coös County Family Health Services (CCFHS) seeks compassionate, dedicated healthcare providers to join its mission-driven team at a Federally Qualified Health Center (FQHC). Providers will deliver high-quality, accessible primary and specialty care to diverse patients, regardless of ability to pay.

Key responsibilities

• Diagnose and treat acute and chronic illnesses, provide preventive services, screenings, and health education.
• Assess patient needs for social and nutrition services and coordinate referrals to specialty medical and behavioral health providers.

Required profile

• Licensed physicians (MD), Nurse Practitioners, Physician Assistants, Dentists, Behavioral Health Providers, or other licensed clinicians.
• Commitment to patient‑centered, team‑based care in an underserved, rural community.

Required skills

• Evidence‑based clinical practice.
• Ability to work collaboratively within an integrated care model.
